Schedule Now for Updated Asthma and Food Allergy School Forms
As the end of the school year approaches, now is the time to think ahead for the 2026-27 school year. If your child has asthma, food allergies or both, updated school action plans are required before the first day of school, and summer appointments fill up fast.
What Forms Does My Child Need?
Most Minnesota schools require one or both of the following:
- Asthma Action Plan outlines your child’s asthma triggers, daily medications and steps to take during a flare-up. School nurses use this plan to keep your child safe during the school day.
- Food Allergy and Anaphylaxis Emergency Care Plan (FARE Form) documents your child’s food allergies, symptoms to watch for and emergency response steps including epinephrine use.
Both forms must be completed and signed by your child’s allergist and updated each school year.
Why Schedule Now?
Scheduling a summer visit gives your provider time to review your child’s current symptoms, update medications if needed and complete all required forms before the school year begins. Waiting until August means competing with many other families for the same appointment slots.
Important: Annual Visit Required
If your child has not been seen within the past 12 months, we are no longer able to complete school forms without an appointment. This policy ensures school nurses have the most accurate and up-to-date care plans for your child.
What to Expect at Your Visit
At your summer appointment, your provider will:
- Review your child’s current asthma control or food allergy status
- Update any medications or emergency plans as needed
- Complete and sign all required school forms
- Answer any questions you have heading into the new school year
